CSI Incident Reporting Form

Terms, Conditions, and Waiver for CSI Intake Submissions

Terms and Conditions

CSI is not a law enforcement agency. Rather, CSI acts as a single point of contact for critical incident coordination, information and intelligence sharing, safety and security training, and resources for Jewish institutions across Los Angeles. CSI does not act as your legal representative and makes no promises as to the outcome of your Inquiry and the reporting of any incident set forth therein. CSI will evaluate the incident reported in your Inquiry and determine whether further action is appropriate. In submitting an Inquiry, CSI collects your personal information. This information is used for investigative purposes, referrals to law enforcement, communal security threat assessments, security-related responses, and CSI’s own business purposes (such as contacting you at a later date or sending notifications to you, among other items). CSI may also use personal information to maintain internal record keeping and to analyze user behavior. CSI is committed to securing data received with Inquiries and keeping it confidential and follows generally accepted standards to protect personal information submitted, both during transmission and once received. Data/information received by CSI is retained indefinitely and used for internal purposes. CSI will never sell your data/information, nor share your data/information with a third party, other than law enforcement or members of the Southern California Jewish Security Alliance, without your written consent. When appropriate for an investigation or when requested by law enforcement to start an investigation, CSI will share data/information with appropriate law enforcement agencies to investigate or further analyze the incident you are reporting in your Inquiry. CSI will also share data/information obtained from Inquiries as required by law, such as to comply with a subpoena or legal proceeding. If you are not a client Point of Contact (POC) in CSI’s database, CSI can only provide very limited information to you about the reporting site. We will directly reach out to a reporting site POC if more information is needed.

Waiver

By agreeing to these Terms and Conditions set forth herein, and submitting the Inquiry to CSI, you acknowledge and agree that: You have read and acknowledged the Terms and Conditions, and acknowledge that the activities of CSI and Jewish Federation Los Angeles (“the Federation”), and their past and present directors, officers, employees, representatives, and related entities, are advisory only and that CSI and the Federation are not acting as legal counsel or as a legal representative. By submitting your Inquiry, neither CSI nor the Federation has made or is making any promise or guarantee that specific action will be taken with regard to the Inquiry and incident, events, or circumstances reported therein. To the extent any legal action is required due to the incident, events, or circumstances set forth in the Inquiry, it is solely your responsibility to initiate such legal action. CSI and the Federation are not responsible or liable under any circumstances for loss, damage, or injury which you or others may sustain due to the submission of the Inquiry, the incident, events, or circumstances set forth in the Inquiry, and CSI’s involvement in the Inquiry. You and your heirs, successors, transferees, assigns, and related entities hereby RELEASE AND DISCHARGE CSI and the Federation, and their members, directors, trustees, officers, employees, contractors, agents, affiliates, agencies, successors, and assigns (collectively “Releasees”) from any and all claims, actions, causes of action, and demands of every kind and nature in law, equity, or otherwise, known or unknown, suspected or unsuspected, disclosed or undisclosed, for damages, losses, liabilities, costs and expenses, actual or consequential, past, present, and future, arising out of or in any way related to submission of the Inquiry to CSI and the incident, events, and circumstances set forth in the Inquiry, and any action or non-action by CSI with regard to the Inquiry.
